Details required related to COVID -19 Vaccinations Coverage in Commercial Establishments.
Dear all,
Your association SIMA is forwarding herewith a link to Google Form regarding the details related to COVID -19 Vaccinations of industrial employees/workers.
Through the Google Form, the department seeks to obtain the details regarding the Covid-19 vaccination coverage in the industries. The Department has shared a google sheet in which all the industries have to fill up their details of the company like name, address, name of the HR head or the person in charge of employees' vaccination, number of employees vaccinated with 1st or 2nd dose of vaccine. This will help the department to find out the number of people vaccinated in the territory and will also help in further vaccination.
So, all the colleague industries are requested to fill in the required information in the google sheet by Sunday evening.
