Claim Form No. 31 - Instructions on submission of Declaration Form therewith

Dear all,

Your association SIMA is forwarding the Circular No. WSU/10(1)2011/ Changes in MAP/13202 dated 26th August 2016, which has been issued by Employees' Provident Fund Organisation in respect of Claim Form No. 31-Instructions on submission of Declaration Form therewith. The scanned copy of the circular is attached herewith.

It had been clarified vide Circular dtd: 04/08/2016 that, the Declaration Form can be accepted in case of UAN based Claim Form No. 31(UAN) without attestation of the employer. The same procedure may also be adopted for Claim Form No. 31 also. So, the attestation for the employer on the Declaration form would not be required for both Form No. 31 & Form No. 31(UAN). However, the Claim Form No. 31 shall continue to be attested by the employer.
